Unsafe behaviors or unsafe decisions are usually contributing factors in accidents that lead to injuries on the job site; in fact, unsafe behaviors, on average, account for 90% of all accidents on a project. Sometimes an unsafe behavior increases the severity of injuries; other times, it is simply a key determinant in causing the incident.
